POLICIES, PROCEDURES AND NOTIFICATIONS OF
PERRYSBURG DANCE ACADEMY

SAFETY 

Parents are responsible for their child(ren)'s behavior and safety while on our premises, including parking lots, bathrooms, waiting areas, etc. This includes children in your care that are not necessarily dancing with us (e.g. siblings, cousins, etc.). By signing this you are stating that you understand that your child(ren) is/are not permitted in studio classrooms when not participating in the class that is in progress. Children are not allowed in any area of the premises unattended at any time. 

 

Parents are responsible for purchasing proper footwear, which may include more than one pair of dance shoes, and proper dance wear, accessories, and equipment necessary for safety, uniformity, and success in class.  

 

By signing this agreement I agree that I/my child/my children/family members/guests agree to follow Public Health guidelines and mandates AT ALL TIMES. This may include the use of hand sanitizer, hand washing, the wearing of face masks or face coverings, complying to requests for social distancing and other health safety measures as mandated by the Ohio Department of Health or the Wood County Health Department. I acknowledge that these guidelines and mandates will be followed at all Perrysburg Dance Academy LLC locations and any venue where I/my minor child/my minor children are engaged in activities relating to their attachment to Perrysburg Dance Academy LLC. I further agree to follow any dance studio specific guidelines set forth for the public health of all members, dancers, guests, and employees of Perrysburg Dance Academy. These guidelines will be shared publicly in writing, video, or other posted materials. I agree to assume all ordinary costs involved in following all public health guidelines/mandates for myself/my minor child/my minor children.

REGISTRATION

In order to be registered, you must complete the online registration. All annual studio and team registration fee(s) and first month's tuition must be paid at the time of registration or according to our fee schedule. Note: All registration fees are NON REFUNDABLE.

AGE LIMITS WHEN REGISTERING

For classes with age limits, October 31 is the age eligibility deadline date for class placement for the regular school year; July 1 is the eligibility deadline date for summer class placement.

WITHDRAW NOTIFICATION

Parents needing to withdraw their children from classes at Perrysburg Dance Academy must notify the office in writing (either a typed/handwritten note or an email) clearly stating their intent to drop and the date of change of enrollment or registration. You will continue to be billed for all classes until the office is notified of your intention to withdrawal in writing.  Refunds are not given for classes already paid for. Verbal notice to instructors or staff is not sufficient to release financial obligation.  If you withdraw your dancer from classes after you have paid for your recital costumes, no costume refunds will be given.  You will be notified when then costume has arrived and can be picked up at the front desk. Costumes not claimed within four (4) weeks will be donated to charity.

 

If you drop all classes before September 5th, a full tuition refund will be given. If only some classes are dropped prior to September 5th, the tuition cost for the class(es) dropped will be applied to the following months' tuition.

 

All registration fees are NON REFUNDABLE.

TUITION

FOR THE 2020-2021 Dance year: Tuition is calculated on an annual basis (the "Dance Year" being September - June) and is broken down into eight (8) equal monthly installments for convenience.  NOTE: For the 2020-2021 year ONLY, there will be no monthly tuition fees charged in December or June. 

 

The initial payment is due upon enrollment to hold the student's place in the enrolled class. A student is not considered enrolled in a class until the initial payment has been made and can be "bumped" if the class meets its size limit. Should a customer fail to pay their initial payment at the time of enrollment, the initial payment would be due on September 1st, 2020. All subsequent payments will then be due monthly from October to May of the following calendar year. The final annual payment will be automatically deducted on May 1, 2021, for the 2020-2021 dance year.

 

Summer class, camp, and workshop tuition will be automatically deducted on June 1st of each year. If enrolling after June 1st, tuition will be due with enrollment.

 

Private lesson payments are due at the time of the lesson.  Private lessons not paid for at the time of the lesson will be billed to the student's Jackrabbit account and will be automatically charged to the customer's payment of choice (indicated during registration) the same day or as soon as possible. 

Monthly team tuition from September through June will be billed and is to be paid with all other studio tuition.  All team tuition continues to be billed until the end of their seasons (see your individual team contract for specifics).

 

Note: Some months have three classes while other months have five classes, but the dance year as a whole has a minimum of 30  lessons per class included in the tuition. Depending on the day of the week they fall on, some classes receive more lessons than others. At least one inclement weather day has been built into the class schedule and all holiday time off has been accounted for. If we incur more than one "inclement weather day" for the day of your child's class or an unforeseen event occurs that takes your child's annual class total below  lessons, a make-up day or option will be provided. This may include an online option for make-up classes.

 

By signing this agreement, you acknowledge that the use of ZOOM (tm) or other reputable online service will be employed to provide interactive online or videotaped classes/instruction to Perrysburg Dance Academy LLC's students in lieu of onsite studio classes should Perrysburg Dance Academy LLC be mandated to close by the Governor of Ohio, the Ohio Department of Health, the Wood County Health Department or any other governmental agency with the power to do so.  Neither refunds nor discounts will be given to families deciding to forgo online instruction during mandated closings.

LATE FEES
Any accounts past due on the 20th day of the month are subject to a late fee of $15.00 (October 20 through April). Student's whose accounts fall a month behind on payments will not be permitted to participate in dance class(es) until their account is paid in full (including late fees). Any account with a balance remaining on May 20th will be subject to a $50 late fee. All accounts must be paid in full, including late fees, by May 25th to participate in the Dress Rehearsal and Recital. No costume or recital fees will be refunded.

PRO-RATING & REFUNDING

We do not prorate or refund tuition for any reason except at the very beginning of a student's enrollment with us or in the case of extended, serious injury or illness.

 

If joining a class after the first week of the month, class fees may be prorated. 

 

Parents should contact the office as soon as possible if their student has suffered an extended, serious injury or illness resulting in a student being unable to participate in dance class(es) longer than three consecutive weeks. Requests for prorating or refunding of tuition should be made in writing and accompanied by a doctor's note. 

 

Students who have suffered a serious illness or injury will be required to provide a doctor's note in order to return to dance. We cannot guarantee that we can accommodate physicians orders for "partial return to activities".  In these cases, students may be required to sit and observe the class during the activities not allowed by their physician. 

REGISTRATION & TEAM FEES

The following fees apply to ALL students:

Annual Non-Refundable Registration Fee (billed and due annually at the time of registration/enrollment--see note below):

$15.00 sibling(s)

Maximum per family: $60.00

(No other discounts apply)

 

NOTE: The term "annual" refers to the "Dance Year" which is considered beginning every September.  Students enrolling mid-year do not receive a discount on their registration fee and will be assessed a new registration fee at the time of all subsequent returning years.

 

Non-Refundable Registration TEAM ONLY Fees:

A-Team $50.00 per dancer  (non-refundable, billed and due in September)

Elite Dance Team $100.00 per dancer  (non-refundable, due with signed contract)

Art in Motion $50.00 per dancer  (non-refundable, non-refundable, billed and due in September or at the time of enrollment if after September)

No discounts apply to team fees.

 

For other Perrysburg Dance Academy team-specific obligations (including financial), consult your team contract.

RECITALS & RECITAL FEES  

All studio classes with the exception of adult (ages 17+) and technique-only classes will participate in an annual end-of-the-year spring dance concert typically being presented the first weekend of June. Dancers are not required to participate in the spring dance concert; however, it is an integral part of the dance year.

 

Dancers participating in the Spring Dance Concert will be billed the following fees:

 

Non-Refundable Costume Fee (billed in November and due December 15th)

$80.00 per costume (As of July 2019)

 

Non-Refundable Recital Fee (added to March statement and due with monthly tuition)

$25.00 per dancer

(No discounts apply)

This fee covers the cost of the rental of the recital venue for the use of a run-through and dress rehearsal as well as instructor/staff fees related to these events. 

 

COSTUME FEES WILL BE AUTO-PAID EACH YEAR ON DECEMBER 15TH! Customers may make other arrangements to pay costume fees, but need to do so prior to December 10th. To be considered "on-time", costume fees must be paid in full each year by December 15th. Any costume fees paid after December 15th will incur a $10.00 late fee per costume. Availability of costumes ordered late are not guaranteed! No costumes will be ordered after February 1st.

 

Dancers NOT participating in the spring recital are exempt from both the Costume and Recital Fees; however, it is the parent's responsibility to inform Perrysburg Dance Academy LLC in writing that their child will not be participating in the Spring Dance Concert no later than December 10th.

 

BOTH COSTUME AND RECITAL FEES ARE NON-REFUNDABLE.

DISCOUNTS & SCHOLARSHIPS

MULTI-SIBLING DISCOUNT: There is a 10% sibling discount applied to families/accounts with more than one sibling registered and enrolled in classes with us. This discount applies to all siblings after the first in descending order of tuition (the first student being that with the highest tuition rate). This discount only applies to monthly class tuition. The multiple sibling discount does not apply to annual fees, special camps or classes with a set fee, drop-in classes, private lessons, tickets, costumes, etc. and cannot be combined with the military discount. There must be at least two actively registered and enrolled siblings in order to receive the sibling discount. Should a family drop below two (2) "active" students, the sibling discount would no longer apply to the family's account. Any family with one dancer will pay the posted fee schedule appearing on Perrysburg Dance Academy's website and the Parent Portal. The "sibling" discount does not apply to adult classes.

 

MILITARY DISCOUNT: We offer a 10% Active Duty and current Reserve US military discount. This discount only applies to monthly class tuition of all registered and enrolled immediate family members of the military personnel. The military discount does not apply to annual fees, special camps or classes with a set fee, drop-in classes, private lessons, tickets, costumes, etc. and cannot be combined with the sibling discount. Members must be the student's spouse, biological mother or father, stepmother or stepfather, legal guardian, or the military member themselves. The military member's current US Military ID or Military Reserve ID must be presented at the front desk at the time of registration each year. We may ask that you complete a short form giving us permission (or not) to include your name in a message of gratitude in our annual recital program.

 

SCHOLARSHIPS: 

Annually one (1) winner of the VICKI HARTKOPF TAP ACHIEVEMENT SCHOLARSHIP will be announced at the end-of-the-year awards ceremony. This scholarship is valued at $250.00, cannot be "cashed in" for money, and are non-transferable. It must and can only be used toward monthly tap tuition at Perrysburg Dance Academy the preceding dance year. The winner will be chosen at random from a pool of tap students referred in the month of May to the Studio Director by all studio class tap instructors. The winner must be currently enrolled in a Junior Tap through Tap 6 class at Perrysburg Dance Academy at the time they receive the award. Students who do not return (or return but do not re-enroll in a tap class) the following year forfeit their scholarship and an alternate winner will be chosen.  Students can receive this award more than once but not in two consecutive years. This is an achievement award based on students mastering stated classroom goals in the area of tap dance.  NOTE: You MUST BE PRESENT (at the end-of-the-year awards ceremony) TO WIN!

 

Annually one (1)  winner of the PATRICIA ALBRIGHT "BRIGHT STAR" AWARD will be announced at the end-of-the-year awards ceremony. This scholarship is valued at $250.00, cannot be "cashed in" for money, and is non-transferable. This scholarship must and can only be used toward monthly class tuition at Perrysburg Dance Academy the preceding dance year. One winner will be chosen at random from a pool of students referred in the month of May to the Studio Director by all studio staff, instructors, demonstrators, and assistants. The winner must be currently enrolled in at least one class at Perrysburg Dance Academy at the time they receive the award. Students who do not return the following year forfeit their scholarship and an alternate winner will be chosen. This is not an achievement award but is based on attributes such as friendliness, kindness, and being of bright and sunny nature. NOTE: You MUST BE PRESENT (at the end-of-the-year awards ceremony) TO WIN!

 

Annually one (1) winner of the SATIN RIBBON SCHOLARSHIP will be announced at the end-of-the-year awards ceremony. This scholarship is valued at $250.00, cannot be "cashed in" for money, and is non-transferable. It must and can only be used toward monthly pointe tuition at Perrysburg Dance Academy the preceding dance year. Nominees will include all dancers currently dancing en pointe at the time of the awards ceremony and chosen by PDA's Ballet Mistress. Winner must be currently enrolled in pointe class at Perrysburg Dance Academy at the time they receive the award. Students who do not return (or return but do not re-enroll pointe class) the following year forfeit their scholarship and an alternate winner will be chosen.  Students can receive this award more than once but not in two consecutive years. This is an achievement award based on students mastering stated classroom goals in the area of ballet/pointe. NOTE: You MUST BE PRESENT (at the end-of-the-year awards ceremony) TO WIN!

 

NOTE: Although unlikely, Perrysburg Dance Academy LLC reserves the right to discontinue discounting or the granting of any or all scholarships at any time.

MISSED CLASSES, MAKE-UP CLASSES & ATTENDANCE

Punctuality and consistent attendance are expected of all students of Perrysburg Dance Academy. 

 

2020-2021 Guidelines for make-up classes: If your child needs to miss a class, he/she can attend another class of the same or similar genre either at or below their own level via the use of online instruction and at home participation.  Each class will be emailed information regarding make-up options in the month of September 2020.

 

Holidays and closures appearing on the studio calendar have already been calculated into your payment schedule and do not qualify for makeup classes.

 

Students must attend the 5 out of the last 6 class periods in order to be prepared to dance in the end-of-the-year recital. At the instructor's digression, any student missing more than one out of the last six lessons the year may be required to take a private lesson at their own expense. This will only be done if it is necessary and in order to make your student ready for the performance.

STUDIO ETIQUETTE & LOST AND STOLEN ITEMS

Students are responsible for their own personal items while at the dance studio. Perrysburg Dance Academy is not responsible for lost or stolen items.  Students should NEVER touch, consume, or otherwise use another person's property (including food). Students should be respectful of the dance studio instructors and staff.  Student should pick up after themselves.  Students should notify staff of anything that looks out of place (e.g. a toilet out of order, etc.).  Bullying, body shaming, gossiping and other behaviors unbecoming of a student or parent of Perrysburg Dance Academy will not be tolerated.  Depending on the severity of the offense, should a student or parent's behavior become a risk or menace to others, the parents will be notified and (in the most severe cases) the student could be asked to leave. 

Chewing gum is NEVER permitted in dance class!  No food or drinks (other than water) are permitted in our studio classrooms.

CLASS SIZE

For safety, comfort and so that students receive the attention they deserve from their instructor, we limit class sizes.  Class size is determined by the style of dance, the common age of the students and the classroom size.  

Perrysburg Dance Academy reserves the right to cancel any class that does not have a minimum of six (6) students enrolled on October 1st of each dance year, or if enrollment drops below six (6) students.  This policy also applies to summer classes that do not meet the five student minimum by July 1 of any given summer session.  If a class is canceled due to insufficient enrollment, an alternate class will be offered to those who are enrolled in said class or a refund issued for the unused portion of tuition.

UPDATE: Due to Covid 19 guidelines the following additional classroom size provisions have been implemented:

Class sizes will be strictly enforced! Perrysburg Dance Academy will adhere to the guidelines set forth by the Ohio Department of Health which allows for 1 person per 100 square feet in gyms and dance studios. Studio A will allow for no more than 8 persons total in the room at one time. Studio B will allow for no more than 18 persons total in the room at one time. These limits include ALL persons (instructors, assistants, demonstrators, dancers, etc.). In order to visualize and make social distancing easier, we have taped squares for our dancers allowing for no more than 6 dancers in Studio A, 15 dancers in Studio B, and 12 dancers in our Annex. While guidelines as of July 26, 2020, allow for this number of dancers, we feel it is safest to limit class sizes to 10 participants in Studio B and the Annex to ensure that classes will not need to be cancelled in the event State guidelines tighten.

You are not authorized to display or use the Marks in any manner without the express prior written consent of Perrysburg Dance Academy. The use or misuse of the Marks or other trademarks, names, logos, service marks and/or trade dress or any other materials contained herein, except as permitted herein, is expressly prohibited.
